Fill out Eastwood Facility Waiver

This needs to be filled out after a player registers for our program and before a player can participate at our facility. Please download the document, sign it (or e-sign), and then upload it. Please attach a photo of your insurance card as well. 

This document does NOT count as registration, but please note a players registration is not complete without this document.
